Regarding identity verification documents when applying for a certificate

As the transitional period for the transition from "health insurance cards" to "My Number health insurance cards" will end on December 1, 2025, "health insurance cards" will no longer be required as identification documents to be submitted when applying for a certificate.

After Tuesday, December 2, 2025, this document will no longer be accepted as identification.。

From now on, the following will be considered valid identification documents, so please keep this in mind when applying.
・My Number Card
・Qualification confirmation letter
・Driver's license
·passport
・Pension certificate, pension book
