[NST Club Activities] Learning about "Rehabilitation and Nutrition" from a Physical Therapist

― 運動と栄養の両面から患者さんの回復を考える ―
9月30日、NSTクラブでは、兵庫医科大学病院 リハビリテーション技術部の理学療法士・梶原和久さんを講師にお迎えし、「リハビリと栄養」をテーマとした講義および症例検討を実施しました。今回の企画は、NSTクラブとして初めて“理学療法士”を講師に迎えた取り組みです。

In addition to working as a physical therapist at Hyogo College of Medicine Hospital, Ms. Kajiwara is also a member of the development team for "Osteofrail Pilates," a method specifically designed for preventing osteoporosis and frailty, which was jointly developed by various medical professionals including orthopedic surgeons, physical therapists, registered dietitians, and dental hygienists.

― 運動と栄養、休養を一体として捉える ―
前半の講義では、まず「筋肉はどのようにつくられ、また、どのように失われていくのか」という基本的な仕組みから学びました。梶原さんは、筋肉を維持・増加させるためには運動による刺激だけでなく、筋肉の材料となる栄養素を適切に摂取すること、十分な休養(睡眠)が重要であり、“運動-栄養-休養”を一体として捉える視点について、学生にも分かりやすく解説してくださいました。

Furthermore, the session introduced the issue of "muscle loss due to inactivity," something we don't often think about in our daily lives. In particular, for hospitalized patients, muscle mass can decrease due to a combination of factors, including not only the disease itself but also rest, reduced activity levels, surgery, and inflammation. We learned the importance of considering "what to eat" and "how to move" together to protect muscles.

―「栄養状態を良くする」その先をめざす ―
講義の中では、「リハビリテーション栄養」という考え方についても紹介されました。これは、患者さんの身体機能を単に回復させるだけでなく、栄養状態を改善しながらリハビリテーションを進め、最終的に患者さんが自分らしく生活できる状態を目指していくという考え方です。必要なエネルギー量をどのように考えるのか、基礎代謝量や活動量などを踏まえてエネルギー消費量を捉える方法についても学びました。

For the students of the NST club, who usually view patients primarily from a nutritional perspective, the idea of considering nutritional management not just as an end in itself, but also in terms of "eating," "moving," and "living," was a valuable learning experience.

― <症例検討>NSTとして何ができるか ―
後半は、重度の熱傷患者を症例として取り上げ、症例検討を行いました。学生たちは、提示された症例について、NSTメンバーとしてどのような関わりができるのか、栄養管理を行ううえでどのような点に注意すべきなのかを看護・栄養・歯科の各視点からグループで検討。

Faced with the unique condition of burns, it's not simply a matter of "supplementing necessary nutrients," but rather about considering appropriate interventions by taking a multifaceted view of the patient's condition. We also received advice unique to physical therapists, such as positioning patients during meals and encouraging them to try what they can do themselves while providing assistance for difficult tasks. It was a practical learning experience that truly emphasized multidisciplinary collaboration, taking into account the status of rehabilitation and physical activity.

Finally, Ms. Kajiwara gave us a warm message: "It is important to think about 'nutrition and rehabilitation' together. Your important role is to connect 'being able to eat' to 'being able to move,' and then to 'being able to move' to 'living' beyond that. 'Nutrition and rehabilitation' are essential perspectives for supporting patients' lives beyond this point. Please continue to deepen your learning about nutrition and exercise at the NST Club."

NSTクラブにとって、初めて理学療法士から直接学ぶ機会となった今回のイベント。参加した国際看護学部3年生の渡邉芽衣さんは「これまでの実習では、術後合併症や筋力低下の予防のため、歩行やベッド上でのリハビリを勧めてきました。今回の講義を通して、筋肉の維持には運動だけでなく、“一人ひとりに合った”バランスのよい食事や十分な睡眠も重要であることを学びました。また、症例検討では、専門分野によって着目する点や意見が異なることを実感し、多様な視点を取り入れることの大切さを知り、今後も多職種で学び合い、患者さんにとってより良い支援を考えていきたいです。」と今回の学びを振り返っていました。

The NST Club will continue to provide opportunities to learn from professionals in various fields and to develop the interprofessional collaboration skills necessary in clinical settings.
